Notes: Headnote: Hypothyroidism is pathological state caused by deficiency of thyroid hormones. Many factors influence synthesis of thyroid hormones, particularly levels of hormones of hypothalamus and pituitary, TRH and TSH are important. Because of that, symptoms of hypothyroidism may significantly vary depending on combination of altered functions of upstream regulators of these hormones. Secondary (central) hypothyroidism accounts for less than 10% of hypothyroidism cases. The causes of it could be a trauma, cancer, other hormones that affect pituitary, or molecular genetic causes (see TSH Release pathway). TRH stimulation testing is used for the differential diagnostics of secondary (pituitary disorder) and tertiary (hypothalamic disorder) hypothyroidism. In hypothyroidism due to pituitary disorder, administration of TRH does not lead to increase in TSH level.
