Tremors are unilateral, occur at a frequency between 4 and 6 Hz, and almost always are prominent in the distal part of an extremity. Hand tremors are described as supination–pronation (“pill-rolling”) tremors that spread from one hand to the other. Rest tremor in patients with PD can also involve the lips, chin, jaw and legs but, unlike essential tremor, rarely involves the neck/head or voice. Thus a patient who presents with head tremor most likely has essential tremor, cervical dystonia, or both, rather than PD.
