Luders 1998 Dialeptic Seizure Location: Left Hemisphere/Right Hemisphere “Dialeptic” seizures have as their main ictal manifestations an alteration of consciousness that is independent of ictal EEG manifestations. The new term “dialeptic” seizure has been coined to differentiate this concept from absence seizures (dialeptic seizures with a generalized ictal EEG) and complex partial seizures (dialeptic seizures with a focal ictal EEG). Dialeptic seizures is a new term coined to identify seizures in which the predominant symptomatology consists of an alteration of consciousness (see discussion of terminology above). Alteration of consciousness is difficult to define. However, for the purpose of identifying dialeptic seizures, we classify altered consciousness as does the ILAE classification of epileptic electroclinical complexes, e.g., as episodes of unresponsiveness or decreased responsiveness that are not caused by motor alterations. Dialeptic seizures are associated with complete or at least partial amnesia for the episode; therefore, amnesia of the episode is necessary to establish the diagnosis of dialeptic seizure.
