Watershed (or borderzone) infarcts represent about 5% of all strokes. They involve the junction of distal regions of two arterial systems. WS infarcts have been studied best in the anterior circulation in relationship to severe stenosis or occlusion of the ICA. Two typical patterns are observed: cortical WS (CWS) and the internal WS (IWS) strokes. The CWS area is located superficially in the cortex between the MCA, ACA and PCA territories. Strokes appear radiologically as wedges extending from the prefrontal or parieto-occipital cortex down to the frontal and occipital horns of the lateral ventricle respectively. The IWS area is situated in an anterior–posterior orientation in the centrum semiovale and along the lateral ventricle
